Change Delivery Preference
By going paperless you are required to provide one email address, though you may provide two. We will use this email address to send you notifications informing you when your forms become available on this site.
Step 1: Your consent required
By checking this box, you are confirming that you understand the following:
- An email notification will be sent to your preferred email address when your Form 1095 is available on the site, and you will no longer be sent a paper copy through US Mail.
- If this email notification is returned undeliverable, you will be mailed a paper copy, but your delivery preference will not be changed.
- Your election is effective immediately, applicable to all Form 1095s issued on or after today’s date regardless of tax year.
- Your election will remain in effect indefinitely, even if you terminate employment.
- You will continue to have access to print copies of your Form 1095, or request a copy be mailed to your specified address, without changing your delivery preference. Online copies are retained for 2 years.
- You may withdraw consent at any time by changing your delivery preference on the Profile tab of this site.
- You may provide or change your alternate email address at any time on the Profile tab of this site.
- Please view the System Requirements using the link below.
Please check the box to immediately enroll in electronic delivery; if you do not, you will continue to receive a paper Form 1095 via US Mail.
