Make Changes

This screen displays your approved beneficiaries, which are your valid beneficiaries. A "requested" beneficiary is an unapproved beneficiary designation that may require additional documentation or written or notarized signatures and for which a completed and signed Beneficiary Designation Authorization Form has not yet been received and approved by the Iron Mountain Benefits Center.

Only approved beneficiaries will be recognized in the event of your death. To add a new beneficiary, change an existing beneficiary or to review requested but not yet approved beneficiaries, click the Make Changes button.

"Evenly Distribute" indicates that the remaining available benefit percentage will be evenly divided among the selected beneficiaries.

Note: For display purposes when printing, yellow highlighted areas are noted by an exclamation point (!) on Printer Friendly pages.
