Access Page Help for additional information.
Update allocations for beneficiaries you have already assigned to a plan.
Review or update a beneficiary’s information by clicking on the beneficiary’s name.
Add a new entity to your beneficiary designations by clicking the Change Beneficiary button.
When you have completed your updates, click the Submit button to continue or the Cancel button to reset the page without saving any changes.

This screen displays approved and requested beneficiaries by plan. "Approved" beneficiaries are designations that have been validated and accepted. In some cases, a signed Beneficiary Designation Authorization Form must be submitted. "Requested" beneficiaries, highlighted in yellow, are beneficiaries for whom information has been entered, but a completed, signed Beneficiary Designation Authorization Form is required and has not yet been received by the CTA Benefits Center. Only "approved" beneficiaries will be recognized in the event of your death.
