COBRA
Let's get started!
For more information about eligibility rules, see your Summary Plan Description.
You can enroll your spouse/domestic partner and other eligible dependents, too (even if they did not receive coverage while you were an active employee).
If you are eligible, you will receive COBRA and Retiree Medical plan enrollment information in the mail, and you will have 60 days from your last day of work to make your election.
Your payment is due on the first day of the month for which coverage is to be provided. For example, contributions for April coverage are due on April 1.
A 60-day grace period is applied. If payment is not received within your grace period, your coverage will cancel retroactively to the last day of the month in which coverage was paid in full.
Option 1: To set up direct debit so your monthly payment is automatically deducted from your personal checking or savings account, click here.
Option 2: Pay by check. Make checks payable to: ABC Company Benefits Center
Payment should be sent directly to:
ABC Benefit Center
123 Main St
Anytown, NJ 08003
Once coverage is canceled for non-payment or underpayment, it cannot be reinstated. Full premium payments must be postmarked by the grace period end date.
If you have any questions or concerns about how to pay for your Retiree Medical coverage, call ABC Benefit Center at 800-888-8888. Benefit Specialists are available Monday through Friday, 8 a.m. to 6 p.m. Eastern Time on all business days.
